What Is the Vaccination Exemption Form?
The Ontario Ministry of Health provides a specific document that parents must complete if they are seeking a non-medical exemption from routine childhood vaccinations. The form is called the Immunization of School Pupils Act (ISPA) – Statement of Conscience or Religious Belief Affidavit.
This document must be printed, completed, and signed in the presence of a notary public before it is submitted.
What You Need to Bring to Your Appointment

Important Notes
Do not sign the form before your appointment. The signature must be done in front of the notary to be legally valid.
If both parents or guardians are required to sign the form, they must both be present at the time of notarization.
